Heritage Family Practice participates with many insurance plans. A partial list is provided below:

Innovation Health
Anthem BCBS
United Health Care
Optimum Choice
Cigna (this excludes the Cigna Connect Network)
Southern Health

HFP does not participate in any Medicaid plan. This includes plans managed by traditional insurers such as UHC and BCBS.

An important note about insurance: All major insurance companies offer multiple networks and products within those networks. Many of those plans are managed by individual payers, such as Unions, large self funded employers and municipalities. Those entities process and pay claims but do not contract directly with physician offices. They buy the rights to use physician networks contracted by larger, traditional Insurance companies like Aetna, UHC, and BCBS. We contract directly with these entities. For example, your card may say Innovation Health, However, Innovation is actually an Aetna product. Insurers are constantly changing their offerings and often develop “Narrow” or “closed” networks (such as Cigna Connect). Unless we are offered participation in these types of networks, it is difficult to keep track of all the options. It is not always easy to answer the question “do you participate with my insurance.” We understand this can be frustrating. It is for us too.

Medicare Advantage: These plans are even harder to track. Many traditional insurers offer Medicare plans under their own names as Medicare Advantage plans. As Medicare Participating providers we are often automatically included in these networks. However, some plans require physicians to sign specific contracts related to these plans. If you have a Medicare Advantage plan that is new to us, we may not be able to answer the question “do you participate” without looking into that specific plan.

If you are unsure or have questions about our participation, the best thing to do is call our office and ask to speak with someone in our billing department. We will work with you to determine if your plan is one we take.
